If your furry family is attending you must fill out the Picnic Waiver Form and bring it with you on the day of the Picnic!

Where: Buckeye Woods Park, 6335 Wedgewood Road, Medina, Ohio 44256

By submitting this information, I acknowledge that I have read and completely understand the rules presented and agree to abide by them. I also waive all of rights to claim against or take any legal action against the Medina, OH Camp Grounds, Kim Murray for any injury of any kind to the undersigned or to the Dog or to owner(s), before, during, after or as a result of the picnic taking place on Saturday, September 22th, 2018. I have read the waiver and agree to its contents fully.

Please Note:

Due to the large number of dogs attending the event, we require that all dogs be leashed. This is for the safety of everyone and especially the children that will be attending with their families.

Thank you for your support! We couldn't do it without your attendance and our many friends that volunteer each year. Not only do our friends work hard the day of the picnic but they are busy bees throughout the year gathering donations, stuffing welcome bags, and creating the famous raffle baskets that everyone prays to win the day of the picnic. A big Kudos to each of them for helping us make each picnic memorable.
